PLEASE READ THE INSTRUCTIONS BELOW If you are 18 and under, you will need to print and  complete the parental consent form and bring it with you to the camp you are  working.  It must be signed by your parent or guardian.  It can be downloaded  by clicking HERE. After submitting  this form, you will automatically be directed to the worker screening form which  must be completed also.   You can not work camp unless you have completed the worker screening form!
